We Accept Most Insurance Carriers

However, we are currently closed to the following insurances for NEW patients:  

  • Medicaid: ProviderOne
  • AmeriGroup
  • Coordinated Care Corporation
  • Molina Healthy Options
  • United Healthcare Community Plan

We are out of network for Humana, the PHCS network and MultiPlan. 


We are no longer contracted with or accepting Tricare.

Please have your insurance card with you at the time of your visit.

You Are Responsible For Your Co-Pay At The Time Of Your Visit

If you do not pay your copay at the time of the visit, a missed copay charge will be assessed to your account. 


Any deductibles, coinsurances or non-covered charges will be billed to your address on file after the insurance has processed the claim.

If we are not an in-network provider for your insurance, if you do not have health insurance or we do not file the claim with your insurance company, you will be responsible for the entire charge at the time of service.
